Welcome

As we age, health concerns and safety often dominate care concerns within families, and we often neglect to look at the whole picture. Think of our lives as a jigsaw puzzle – put together with many pieces interlocking to form a single complete picture. Holistic End of Life Planning can assist you in recognizing all the important pieces in the puzzle of your life. Providing ‘person-centered care’ and ‘whole-person care’ through education, advocacy, communication, and empowerment, is the mission of my private social work practice.

Do you need HELP?

Holistic Assessment and Consultation will provide HELP with identifying and addressing the non-medical psychological, social, spiritual, and practical aspects of your life that are of importance to you and your family. I can work with you and your family in planning for your final years, at the time of an anticipated death, sudden or traumatic death and/or following the death of a loved one.

End of Life Planning will provide HELP with preparation of: an Advance Care Plan; education relating to the importance of Powers of Attorney for Personal Care and for Property (finances); education relating to the importance of drafting a Last Will and Testament; and education relating to what should be discussed within your family In relation to your final wishes, funeral plans and the disposition of your body at death.

Life is not over until you physically die. You deserve to live life with quality, dignity and respect … to the fullest potential possible. We can HELP review your bucket list and keep your dreams alive.

Planning for: family conversations, when you are no longer safe to be at home alone, keeping your mind active, decisions around the distribution of assets before and after death, building a legacy project, companionship visits and other needs.
